چکیده

Introduction Percutaneous mitral balloon valvuloplasty (PMBV) is the treatment of choice for significant mitral stenosis (MS). The inability to cross the mitral valve accounts for a non-negligible rate of unsuccessful PMBV. This may be caused by unfavorable septal puncture or extensive enlargement of the left atrium (LA), both resulting in insufficient support. Advanced valvular and subvalvular degeneration may further impede valve crossing. Surprisingly, only a few reports focus on this problem. In a study by Feldman, failure to cross the mitral valve occurred in 1.7% of cases [1]. Cribier et al. reported the occurrence of this failure at 2.6% [2]. We present a case of a successful re-attempt of PMBV done with the support of a veno-arterial loop after an unsuccessful mitral valve crossing with an Inoue balloon in the first procedure.

SHORT TERM CLINICAL OUTCOME OF PERCUTANEOUS BALLOON MITRAL VALVULOPLASTY VERSUS SURGICAL CLOSED MITRAL COMMISSUROTOMY

BaIlon mitral valvuloplasty and closed mitral valve conunissurotomy were performed in 450 and 127 patients, respectively. Also, 254 patients with severe mitral stenosis were enrolled in a prospective randomized trial comparing the two procedures. Immediately after balloon mitral valvuloplasty, mean left atrial pressure was 11.38±3.54 mmHg, transmitral valve gradient was 1.8±2 mmHg, and mit...

Immediate Results of Percutaneous Mitral Balloon Valvuloplasty in Patients with Mitral Stenosis

Introduction: Mitral stenosis is a prevalent valvular disease in developing countries. Percutaneous mitral balloon valvuloplasty (PMBV) is the gold standard treatment. The main objective of this study was to assess the initial results of PMBV in patients with mitral stenosis during 16 years (2002-2018) in Mashhad, Iran. Materials and Methods: From April 2002 to March 2018, 770 patients underwen...

Homograft - the optimal aortic valve substitute?

valvuloplasty can be performed safely and with satisfactory results in over 85% of these patients. It is also possible that repeat PMV can be performed in these patients with acceptable risks and results. In our own single centre experience in about 390 procedures repeat percutaneous balloon valvuloplasty was performed in 17 cases (mean age 65 13 years) without any increase in the rate of acute...

Progressive improvement in pulmonary vascular resistance after percutaneous mitral valvuloplasty.

Percutaneous mitral valvuloplasty has been proposed as a nonsurgical technique for treating high-risk patients with mitral stenosis who are deferred from mitral valve replacement. The effect of this technique on patients with pulmonary hypertension, however, has not been fully evaluated. Accordingly, serial assessment of pulmonary vascular resistance was made in 14 patients with critical mitral...

Right Atrial Clot Formation Early after Percutaneous Mitral Balloon Valvuloplasty

Mitral balloon valvuloplasty which has been used for the treatment of rheumatic mitral stenosis (MS) for several decades can cause serious complications. Herein, we presented right atrial clot formation early after percutaneous mitral balloon valvuloplasty which was treated successfully with unfractioned heparin infusion.
